If you're forwarding an email you received that had attachments, you should be presented the choice of including or not including them after you tap the forward button.

Hmm, might be some kind of bug. If that's the case, try copying the image first, create the new forward message, and paste it in. See if that works.

Does this happen with all attempts to forward emails with attachments, or only this one? There is a possibility the image file has some kind of corruption that causes iPhone to "forget" that it's supposed to be attached. I've had such things happen in other programs before, where a file that seems just fine is actually corrupted and isn't recognized or is recognized improperly.

Copying and Pasting is basically creating a new hidden image stored in memory from the picture data of the original. Doing this can stamp out any such corruption.

I had this happen today actually. Tried dragging a PNG image from Safari to iChat. Instead of sending it as a picture as normally happens, it kept trying to send it as a file transfer. Opening the image in Preview and saving a new copy solved the problem. Perhaps the same sort of thing is going on here?

come to think of it sending and saving pictures are kind of similar.
